Designed and built by a Physiotherapist who knows where the transition from school goes wrong.

Four young people walking together along a sunny campus path, one using a wheelchair with a communication device and one using crutches.

What's in the app

Built for young people aged 16 and over who are approaching the end of school, and the parents, carers and supporters who help them.

A checklist that keeps pace

One clear task each week, set around when you expect to finish school. Fall behind and it helps you catch up; get ahead and you can browse what's coming.

Learn

Short, practical articles linked to your checklist tasks, written in plain English, to help with each step of the move from school.

Practical tools

Prepare for appointments and keep a record of your equipment, then turn what you've entered into a document to share.

Plan together

Link with a parent, carer or trusted adult so they can see your checklist progress. You choose what's shared, and you can stop at any time.

Community

A moderated space to share experiences and ideas with others going through the same move.

Private by design

Health, disability and NDIS details are only kept if you say yes. Discreet mode, Easy Read summaries and plain-English choices throughout.

Made by someone who's seen the gaps

Transition Guide was designed and built by a Physiotherapist who has watched too many young people with disability reach the end of school without a plan. Equipment reviews get missed. Funding and services lapse at the worst possible time. Adult health and support services are hard to find. Families are left to piece it all together on their own, often only learning what they needed to do once it's too late.

Transition Guide turns that hard-won experience into one clear, step-by-step plan: the right things, in the right order, at the right time. Each task is paced to when you expect to finish school, so nothing important slips through the cracks and nothing feels overwhelming.

Transition Guide provides general information and planning tools. It isn't a health service and doesn't replace advice from your own health professionals.

Simple pricing

A$9.99 a month for your first 6 months, then A$14.99 a month

One subscription covers you and the person you link with. If you have an NDIS plan, the app may be able to be paid for from your plan funding where it's reasonable and necessary for you. Your plan manager or support coordinator can tell you. For NDIS plans, we send an invoice after each month.

For health practitioners, teachers and support coordinators

Transition Guide gives the young people you work with a structured, step-by-step plan for leaving school, and lets their family or trusted adult follow along.

Built by a clinician

Designed by a Physiotherapist, drawing on first-hand experience of the transition from paediatric to adult services, and of the steps that are most often missed.

Recommending the app

Simply let your client know about Transition Guide. We don't ask you for any information about them, and our materials don't collect any.

NDIS invoicing

For self-managed and plan-managed participants we invoice monthly, after each month, with the participant's details, NDIS number and support item.

Information only

Transition Guide provides general information and organising tools. It doesn't assess, diagnose or treat anyone, and doesn't use artificial intelligence.
